ISMP Symposia at the 2016
ASHP Midyear Clinical Meeting

The Institute for Safe Medication Practices (ISMP) is an independent, nonprofit charitable organization that provides education about medication errors and their prevention. The Institute has more than 40 years of experience in helping healthcare practitioners keep patients safe, and continues to lead efforts to improve the medication use process.

There will be a wide variety of opportunities to learn from ISMP experts at the 51st annual ASHP Midyear Clinical Meeting in December. Below is a list of ISMP symposia that will be conducted.

This symposium will provide you with key information on how to prepare your organization for auto-programming of IV infusions and optimization of closed-loop technologies. An overview of the planning and implementation process in two different EHR systems will be provided, paying particular attention to the human factors, technology readiness, environment, and system challenges faced during the process.

This symposium will highlight the experience of one pharmacy director who was challenged to respond to a fatal compounding error and led his team through a time of uncertainty and transformational change. Other expert speakers will address risk points and risk reduction strategies, explore the use of technology to augment manual processes for preparation and verification of sterile products, and highlight changes made in the recently updated ISMP guidelines for sterile compounding.
