Activity Description

Patients living with HIV, Hepatitis C and other infectious diseases sometimes require specialty medications to manage or treat their conditions. These medications are often difficult for patients to afford and often cause challenging side effects. Without appropriate support from their pharmacy, patients may struggle to adhere to their therapy regimens, resulting in subpar clinical outcomes.

The clinically-integrated health system specialty pharmacy care model has proven effective at ensuring patients receive the coordinated, high-touch care needed to improve adherence and enhance outcomes. During this webinar, a pharmacist, pharmacy liaison and provider from Summa Health’s Care Center will share:

  • Why clinically-integrated health system specialty pharmacies are uniquely positioned to address the needs of patients in the infectious disease setting
  • How embedded pharmacists and pharmacy liaisons add value to an infectious disease care team
  • What steps a health system specialty pharmacy can take to begin leverage data to enhance care and outcomes for patients living with HIV

Learning Objectives

The target audience for this activity is pharmacists and pharmacy technicians. Upon completion of this activity, participants will be able to:

  • Compare the clinically-integrated specialty pharmacy services care model to other approaches.
  • Explain the roles of an embedded pharmacist and pharmacy liaison on the care team.
  • List metrics that health system specialty pharmacies can track in the infectious disease setting.
  • Discuss how the clinically-integrated care model benefits patients and providers in the infectious diseases setting.
